Touching door knobs, weaving your way through partitions, listening to the jack hammers of the construction crews… and I’ve birthed with women in the intake room, on a very small examination tables, because the other rooms were “closed”. Not FULL, but Closed! Why?

Not enough RN’s that night. Another time, they were on diversion for a different reason, patients were redirected to Seattle!

With hospital cut backs, they are on the “drive through” of services this means you are “in and out” and often you won’t even get in! Sometimes the hospitals close to receiving new clients and put you on a kind of orbiting landing pattern. You get to go in ever day to see how you are doing, then you’ll be sent back home. If you are far enough along and they have no room, you will be re-directed to another hospital…and sometimes different medical teams.

In BC Midwives have rights only at certain hospitals and if you are re-directed you don’t get your midwife, or any of her team!

Now what? You are in labour, you don’t get your team and you are going into a medical facility you know nothing about and you are in labour!

Lets start over, at the beginning:

Hospital births in 1745

The advent of the male obstetrician, or male midwife, began in these early times, and because women preferred their usual midwives, these newly trained medical guys needed a way to get some clients! They began offering poor and homeless women hospital care, free! And this was known as the laying-in hospitals of Dublin. They became popular in other parts of Europe and eventually in the US.

Hospital births were known to be exceedingly dangerous as described in these notes from Da Costa:

*….the crowding in of patients, frequent vaginal examinations and the use of contaminated instruments, dressings and bed linen spread infections in an era when there was little knowledge of antisepsis.

This new disease, “childbed fever” killed approximately 13 percent of women under the care of doctors in the European hospitals in 1846, compared to 2 percent of women under the care of midwives.

This was noted by the doctors of the time, writing up the causes of these deaths, however, there was strong resistance to the idea that childbirth fever could be due to the doctor’s negligence of hygiene. It was towards the end of the 19th century that this was mandated and became part of the routine of care.

Additional marketing on the side of the hospitals and doctors eventually did away with the midwives of ancient times, through negative campaigns and marketing. Eventually it became regulated by governments as the hospitals all are now, and it was deemed illegal to operate or birth without sanction from the governments : regulated licensing and authorization."

Midwifery no longer existed in North America, except for the few renegades willing to take a chance on going to jail, for practicing their Skills and gifts.

Today we have again reached a critical time in birthing. It’s absolutely known the Super bugs that are resistant to antibiotics pose a great threat to new borns and mothers. In addition there are other factors that increase the risks in hospitals that may make home birth a good choice.

Time is money ! The Drive Through of Birthing!

Perhaps the most difficult aspect of birthing is now in the hands of the government and their administrators, yet again. As MBA’s and the advent of the bottom line intercedes on medicine, we enter a new phase where the labouring mother cannot possibly lay in one of the hospitals beds, occupy space, demand we feed her, use our beds and our linens, and our hot water and staff, and then expect us to dote on her every need and whim. Further, her shouts and fearful cries set off other patients that are in hospital for other reasons.

The separation of the labouring mothers from the rest of the hospitals patients was a positive advancement!

Unfortunately, even though the mother and infant are isolated in another wing, not so the staff, the mops, the pails, the cleaning fluids… and the bugs are everywhere!

Go ahead use one of their bathrooms, you want to be sure you have used the sanitizing liquid outside the bathroom when you exit. There are sick people in this hospital! And they are all breathing the same, re-circulated air, using the same hallways, using the same bed pans, and trays for vomit….it’s a shared commodity!

And the special news for the day, a pregnant or birthing woman is not sick, she is just having a baby!

Sterilization of equipment, key when you are in the hospital, and yet the trays get contaminated as people set things down on the wrong trays and then shrug and keep using them. (I've seen it) Use scissors to open a sterilized tray and then use the same scissors for something else. (seen this too)

The birthing rooms are used, one client after another, with very little time to clean, clear and ensure all is disinfected. The rooms are stripped, but the mattress underneath - and the antique pillows....then there is the floor and the mop you see each day wiping it... The bath tubs are sort of clean and there’s no cleaning materials close at hand to do it yourself.

And on it goes…, sadly

In your own home, however, you and your bacteria are already friends!

The medical interventions.

Well, if you are lying there in one of our beds, we’d better be doing something with you!

Birthing is generally like watching paint dry. The mother is in there working, just needs quiet, warmth, and comfort when her muscles are tired. Maybe a hand to the bathroom, maybe some ice chips, water and on she goes. Affirmations.

Uninterrupted, she will bring her baby forth without interventions. These medical interventions, interrupt a massive amount of delicate internal pharmaceutical preparation on the mother and infants bodies and how they are working together.

In the 1970’s it was believed no mother should gain more than 21 pounds any pregnancy. So mothers starved themselves during their pregnancy, and then found after the birth they were some 35 pounds or more less in body weight then when they started the pregnancy. What happened? The baby used the mothers body resources for it’s development. The newly forming infant is the perfect parasite, and will leach the necessary nutrients from mothers body as and when needed. Mothers terrified of gaining weight, sacrificed their own body mass to achieve these low weight gains.

Today we know better, and midwives and doctors generally do not measure a woman’s weight gain nor do they bring it to her attention unless it's required - during pregnancy.

Today we know and accept that breast milk is a finely tuned chemical mix of nutrients and blendings that will so totally support the infant during it’s first months of life, that modern medicine and the pharmaceutical companies no longer throw their weight around on this matter, in the west. They do however, interfere in third world countries offering formula to new mothers until her milk dries up and then they charge her for the formula. This is ethical?

It wasn’t ethical when they gave 99% of women a shot a their birth that dried up their milk and told them, “you don’t need to bother with this nasty stuff anyway, formula is better”.

Today they suspect breast cancer & fibroids or breast lumps are part of a result of “intervention” on behalf of the new mother and infant, a side effect to the mother for later in her life. Hardly supportive as the breast milk of the new mother is filled with exactly the correct balance for this baby. Her body creates exactly what this child needs and will support the development and stimulation of the immune system, through the immature digestive track. And every mothers milk adjusts itself chemically to give her child the exact balance of hormones and nutrients that child needs. Every baby she has, her milk will be slightly different.

Take a look, are we not the largest group of people in history with stomach disorders that are dependent on pharmaceuticals for proper absorption of nutrients & digestion? The advent of stomach remedy’s only slightly less than the advent of formula to feed new borns.

Breast milk thins automatically in the summer so there is no need for adding water, it thickens and is heavier in the winter to provide more fat for adding warmth to baby. It can also be used for things like healing little scratches on baby's new skin, or rashes. Baby's diaper changes are no where near the negative odors, and baby's that are breast fed smell sweet - not like curdled milk gone sour!

It’s the same kind of interventions that interrupt the very delicate balance of chemicals the mother’s own internal pharmacy is working with. If you increase her oxytocin, the brain is intelligent enough to register the increased levels and back off it’s own production. The only reason these drugs work on us, is because our body’s are producing their own supply of them, only ours are without side effects and they are in tune with our body.

A mother on Pitocin Drip is not going to get the breaks in her labour surges, medicated labour comes one after another and just pounds away at her until it’s done. With her own oxytocin her labour moves in a way that is working with her body. She might have several surges in a hour, close together, then a long 30 minute break, then back to steady surges, increasing with intensity. This balance is the result of the body knowing just how, when and how much to release as she and baby work together.

• The slippery Slope of Intervention:

• Ripen the cervix, chemically

• Add Pitocin Drip, rupture the membranes to speed things along.

• Add an Epidural for pain management because the Pitocin Drip is long and hard labour for the mother and less effective than her own would have been but you can’t go back

• The epidural stops the labour for somewhere in the area of hours- lengthening it. Labour backs off, you can see it on the monitoring strip, which mothers also have to be attached to. The medical model now reads the strip and designates “ok, let her rest for a while, then we’ll ramp it up again”

• Baby’s heart rate looks like it’s crashing, when the epidural is added. More reason to do more, to make the process faster.

• Add more pitocin drip to get the labour going

• The epidural is wearing off, add some more, add a baby monitor to the soft spot on baby’s head while still in the vagina, checking on baby.

• The pitocin isn’t working as well, get her to use some of her own PUSHES to help

• The ripening cervix, isn’t “where we’d like it to be”

• Finally we’re close, the mother has lost all confidence and connection to her body and her baby. She has no idea what’s happening in her body at all, she can’t read, monitor or connect to baby or her body. Often she has had several courses of drugs that have long ago passed through the blood/brain barrier of the infant.

• Baby’s head is there, but it appears stuck, call for suction and prepare the mother. The suction cup is placed on the baby’s head, then a nurse will begin to pump the suction. It attaches to the baby, as the doctor pulls and the mother is ordered to push with all her might.

• An episiotomy is necessary now, as the opening is beginning to tear, better to cut than tear…is the pattern of the day. Not always true...but they do as a course of action.

• Incision is made, baby is sucked out, cord is cut instantly and baby is moved to the “safety” of the baby monitoring bed and away from the mother. Baby now has a huge contusion on the top of the scalp a bulge and there is no one in the room that doesn’t think, “that must hurt”…it’s even bleeding and bruising a little. And it’s going to be there for a few weeks.

• The pediatrician now examines and the shocky, stressed baby, filled with drugs who is pronounced “rescued” as it is resuscitated, and wasn’t it a good thing because without all this you would have probably had a damaged infant, even a c-section, or worse.

Now at this rendition, many mothers will say, “but the cord was around his neck, his head was way too big, they said there was no way I could have delivered that baby”

Here’s a different model for consideration:

• After a week of constant pacing, irritating back, tightening tummy muscles, and up every night three or four times, the expectant mother is not that happy with having baby remain where it is much longer.

• The party is over, mothers agitation, her hospitality has been exhausted, there is simply not enough room in this body for me and this baby any longer.

• That inner dialogue and signals begin a series of chemical releases in both baby and mother. The time has come.

• Mothers own body responds with sending chemical messages back and forth, like a relay between mother and infant. She sends out the messages in preparation for the baby to follow her thoughts, her excitement and her own internal dialogue.

• What she sees now, as she begins to visualize baby’s birth, is what baby also begins to see and mirror. If mother sends along the messages from her own birth or her families birth, almost “seeing” baby in the position it will be in for birth, then baby begins to change to that position.

• The infant will follow reading the mothers inner dialogue, feelings, emotions and chemical releases until it gets as close a reading as it can on what to expect next.

• She is preparing the baby for it’s next adventure, just like she has been all through the pregnancy. A terrified mother, is constantly terrifying her infant, her infants brain development will stimulate the reptilian brain to fight, fear, and the stress hormones release...as they prepare for the “battle” that lays ahead.

• If on the other hand, she visualizes baby in the perfect birthing position, with the back to the left side of her body, because this means baby only needs one little turn into the birth canal. Her calm, peaceful acceptance is all the assurance baby needs.

• If midwives and doctors talk about baby positions and give the images, even showing pictures to the mothers about what these positions look like and why they would hurt her so much, ramping up the fear chemicals at the same time, the power of suggestion often does it's work.

• Baby is mirroring all of mothers thoughts, feelings, beliefs, strategies as translated through the neuro-net with the highest content of mirror neurons it will ever have- preparing for it’s biggest experience yet. The infant Mirror Neurons are so strong at the time of birth, because this is the infants survival. It has to be able to take it's signals from mother to survive.

• Baby has been listening to your voice for months. It’s heard and felt the tension when you and your husband argued, it felt the tension when you were over tired, it was still and pensive and you were tight and fearful at the last medical examination. It was receiving all the chemical training it needed to understand when mother sends out these chemicals learning that this is a time for tension, stress & fear.

• When mother tenses up leaving to go to the hospital and begins to fret and send all the negative emotions through her body, her baby goes into fear, and stillness and stress. It’s not safe, and best to be still. Further she restricts her breathing and that blocks the blood flow through the placenta, baby is also quiet.

* The more the stress hormones release the more the tension, the more tension, the more the pain increases, as the muscles begin to fight the process and every surge is an attack on her body that she resists.

• However, knowing fully what is about to happen, this mother knows all this long a week of edgy energy has been early labour preparation. Her baby is coming soon.

• She remains calm, focused and rests. Perhaps she and her husband plan a romantic evening, listening to their favorite music and enjoy each others company together increasing those ecstatic love connections & releasing the bonding hormones between them, even more as they gather their energy to bring in this little one.

• She finds herself impatient and reminds baby, it’s just me, we’re ok.

• She finds herself tired, and takes a nap

• She finds herself stressed with visitors, she asks them to leave.

• She finds herself too busy in her mind to relax, she lays down and listens to a deep guided visualization.

• She rests, and she prepares. In her mind she talks to herself in ways that are supportive and providing instructions to baby.

• “I just want to finish planting the garden before I let you come ok?” Her agenda she knows is key to baby’s well being and baby is listening to her inner voice, her inner mirror neurons are sending baby all the messages, including

I’m ready now. It’s time. Lets do this.

• The cervix begins to “ripen” or thin and open, and the big muscles of the tummy begin to work and to rotate the cervix forward, because up until now, it’s been facing backwards towards the tail bone. And it begins to lift the uterus and lengthen it, and pull up, adjusting the position.
• The big muscles of the tummy are working to lift and move the cervix forward, to help those big muscles, breathe into them, relax them, let them have oxygen and water to do this work!
• Now the cervix is thinning so baby can move through it easily, and as it opens (about 1 cm per hour is the going norm,) (generally using relaxation methods mothers experience about 6-7 hours of labour in total)
• There are three stages to the labour. The 0-10 centimeter thinning and opening phase and the rotation of the cervix forward; the downward movement of the infant, and emerging into the birth canal, and once it's emerged, the birthing of the placenta.

*These are all pliable muscles we’re working with here, they stretch and move easily and you can trust that your body knows just what it’s doing – so let it.

• We have 200,000 years of technology built into our body’s DNA for birthing, and we’re totally capable of birthing any baby our body can build

• Now as the cervix thins fully, and baby actually “transitions” out of the uterus that it’s been living in for all of it’s development, the mother feels new sensations, new pressures on the rectum and on the bladder, as baby's head descends.

• Baby’s head is easily folding along the frontals, as they are specifically created to do. Baby is in perfect position and ready to listen to all mothers directions.

• As mother visualizes baby moving through the birth canal, baby is encouraged to do so. Mother visualizes baby leaving her body...very soon something different starts to happen.

• The surges of the great muscles of the tummy seem to drop back. They aren’t as strong, they are there, but they are not as strong as they were. Baby is out of the uterus, and the uterus is now following baby down. As it does it soon catches up and the next few big surges will bring baby’s head through the birth pathway, out into the world.

• During this time there is a gentle rocking of baby urged forward and then back, it gradually opens the birth canal for itself. Now it feels as if the body is PUSHING and mothers often shout “I’m pushing and I can’t stop”, that’s ok, that’s those big muscles doing it all for you. Just let them, We’re really close now.

• The next feeling can often be of tightening as baby’s head moves through the opening out into the world. It starts slowly we see baby’s head “crowning” and it looks like it has a crown on it, as it emerges slowly.

• Pushing here, or directing a mother to push here, can create a tear. If she will be patient with her body and realize even the tingling is nothing to fear, only just a tightness and as she breathes into this part of her body now, it relaxes fully and softens and opens even further.

• If she’s tight, fearful, upset with the changes, being distracted by the shouting team around her, she will lose the connection. That’s ok, she can get again on the very next surge. Breathing in deeply, giving lots of oxygen to baby and herself, everything calms down. Some of the teams I've worked with say to the mothers "I want to see you much more upset than you are right now"...that's their measuring stick based on other births they have witnessed, but with using hypnosis and gentle relaxation, mothers don't go into this kind of state. They don't need to. They trust their body and their baby and they just release and let go and baby emerges.

It can, and often does leave medical provides in shock and asking "what was she doing, we've never seen anything like that".

• During this transition we now know there is a vital part happening for the infant. The three tiny bones in baby’s ears are part of the trigger as they are squeezed during birth that signal the breathing mechanisms in the brain’s intelligence. Further the squeezing of the chest and the heart as it moves through the birth path also trigger essential brain patterns preparing the infant for it’s transition in the new world outside it’s host mother. The heart begins to pump and draw into the infant body, the blood from the placenta. These are not to be understated, as they are the essential parts of the mechanisms designed by nature to help the child adjust to live in the world. It’s all there, just like tonsils, we didn’t know what they were for, really, so just got rid of them!

• Actually we’re becoming a little more intelligent about accessing mother natures development structures and why things are built they way they are, or work as they do. We know now the triune brain system, is one old system with a new one built on that old one. Mother Nature never starts from scratch, she just improves on the old system by adding a new one, and then increases functionality and adds more new options onto the old one. So we are the representational system of that triune brain system:

• The Reptilian Brain, Fight or Flight, DNA RNA

• The Old Mammalian Brain : Care for the young, emotional limbic functionality

• Pre Frontal Cortex : which now relates with reason, understanding to these older parts of the brain systems, however, an interesting thing occurs in some of these systems.

• The older Reptilian Brain is a fight or flight response system, and when the “startle effect” is engaged, the ability to access the other two areas of the brain are interrupted. That’s why some functions feel like we are at the control of our anxiety instead of able to control it. It’s “beyond reason”, it will not response to reason, understanding or the obvious “well it’s not affecting everyone else the same way”, because they are not in reaction with the unconscious triggers as we are.

• Now baby’s head fully engaged, slips through the opening and the very next surge the shoulders will follow and the rest of the body shortly after.

• This whole process during the transition stage can really be under 20 minutes, and the final transition of baby through the cervix opening and out of mother, is generally about two surges, and that takes approximately 30-90 seconds.

• If however, mother is fearful, resistant, non-trusting of her body and it’s process, not wanting to allow her body to open and release the child, she can hold this part back for hours. Literally, and I’ve seen this too. “what - it’s just going to pop out right now?” We can see the head, and the mother keeps pulling the baby back inside with her fear. Literally for hours, until baby’s stress is too great and we have to help her.

Undoing the fear at the time of the birth is nearly impossible. There is almost no reasoning that will help the mother, she often is beyond her ability to reason and so this is why dealing with all of this early on in the pregnancy can avoid these kinds of issues at the time of birthing.

• “Yes it’s just going to slip out right now, only we’ll use forceps or a suction if you won't allow yourself and your baby to see baby outside of your body, now"

• This is where it’s so important not to become overly excited with the mother. Don’t draw her out in your excitement. I know you have been watching the paint dry for hours and now there’s something happening so the whole place lights up and she gets distracted and comes out of her inner place of working with baby and her breath and her body. Let her finish it. Help her stay calm and focused, relaxing her heart rate and respiratory rate has a direct impact on baby’s heart rate and respiratory rate. These are not two different systems, they are symbiotic in nature, and they affect each other.

• If a baby is in stress and I sense the medical teams getting excited, I direct mom inwards, down, deep, breathing deep to baby and remind her between each surge, because she forgets, she’s tired, she’s not stupid she’s just tired. She’s doing the marathon - baby's heart rate resumes it's correct pattern, as does mom.

• Baby emerges fully.

• Cord around it’s neck or not, is still not breathing on it’s own and there’s no real panic here, although everyone seems to do just that. Panic. Baby is out, mother is catching her breath, baby is birthed, she can relax, and the medical teams get kind of nuts now, because the “baby is in their court”. Now the panic to DO SOMETHING

• The panic to quickly cut the cord, was invented during the war, when speed was of the essence, however, in todays modern drama’s I still think it has to do with our “drive through” mentality. There is no rush! Baby is still on “tap”. Baby can begin to slowly take a first breath, then cough a little, then gradually inhale air for the first time. It can hurt a little! Ever been underwater for a while then burst to the surface and inhaled, it’s a burning feeling! So the cord, and placenta support baby for these first minutes. The cord around the neck is not an issue, just slip it over baby’s head. And midwives do this all the time. Even two or three times around the neck....

• Clamping and cutting before baby is fully engaged robs baby of several things:

Oxygen – resulting in needed resuscitation and additional interventions possible brain damage,

Robs baby of the remainder of it’s blood which is now left behind in the placenta to birth, and a fully engorged placenta is a lot more difficult to detach and birth, if we just let all that blood transfer into baby it will help all the systems and it's the baby's blood! Baby's then are not diagnosed with "low blood volume".

Ever have the doctor tell you that? Well why would that be? "Well we had to cut the cord at birth as it was around baby's neck". Sometimes they can make it sound like it was the mother or the infants fault, when really it just takes a minute to sort it out and then let the process continue.

As the blood transfers to the baby, the mothers system is ramping up her own levels of oxytocin This assists with birthing the placenta, and stopping any bleeding. It also assists with the production of milk, which is just beginning and will be needed for baby soon. If you use synthetic oxytocin here, you are now going to tell mothers own supply to back off the production in her body. And as baby reads mom's mirror neurons, it will also back off it's own stimulation.

You will rob the baby and mother of their intense bonding times together

Baby brain development is vitally dependent on it having all it’s blood and all it’s final and last direct support from mother. And the mothers neurology and internal chemistry releasing it's high levels of oxytocin at birth now stimulate the infant brain through the very powerful Mirror Neurons.

I’ve had midwives and doctors insist on cutting the cord to revive a limp baby. Wait! That infant will do better on it’s mother placenta and near it’s mother – leave it attached and revive the child, waken it on the mother’s body, where it’s warm and stimulating and add oxygen, before you cut it’s primary source.

The medical carts for babies are on WHEELS, they can move to be close to the mother, and all can be done while baby is there within site of her infant!

This is vital life support for the infant brain and new systems coming on line.

• I’ve seen the cord three times wrapped around baby’s neck and a midwife will have this part sorted out before the baby even fully emerges! She just slips it over baby’s head as it makes it’s way out of the birth passage. Baby’s are like little greased piglets! The coating over baby is so thick on their skin they don’t even have little wrinkles on their fingers! It has to be this way, as they have been in water for nine months!

• Which also speaks to the ease for them in emerging, they are slippery!

• Within about 3-5 minutes baby is breathing on its’ own and maybe now it’s time to feel the cord and see if it’s still pulsing. If it is, and don’t clamp it and then say, “see it’s stopped” because the blood in the placenta has to transfer to baby!

• When we interfere at these levels of the birthing stages we interfere with vital processes that are on a very deep level associated to other systems & stages of that infants life, and eventual maturity.

• One of the number one thing new borns are struggling with : abandonment.

• One of the number one complaints on this planet today throughout our experience as humans is the fear of abandonment.

• Where do we structure this in our child? At the moment of birth?

• A few hours later the oxytocin levels in the mother subside, and she begins to mirror different things to her infant. Smiles, loving holding her baby, touching her baby, feeling it’s skin and looking at her infant.

• During the next few days the oxytocin levels will increase for shorter periods as the baby and mother continue their dance of bonding. This is also deeply available to the fathers if they choose to be this connected to their child, and their wife and this experience. And it often can heal earlier feelings of abandonment and separation from their own early beginnings.

• Why do you think we are as distorted about life as we are? Look back along your own history, your own past, where are your first experiences of abandonment? What was your own birth like? Do you know, can you find out?

• People have shared with me the beliefs of their families, the experiences they had as infants and children. “My mother was always afraid of me”….why? What happened to her at your birth? What was she told or thinking about?

At one session an expectant mother told me she didn't find the act of intercourse at all sexy, that fingers did nothing for her. We were talking about birthing and this triggers her experiences with intercourse. Quietly, carefully we worked our way around this, never quite getting into how this had come to be for her, however, eventually the release of emotions allowing her to birth her baby her way and regain some of her personal power.

"My father was never there, he just couldn’t get close." Why, what were his beliefs about himself and his experience as a new life on this planet? Was he loved? Did his birth affect his experiences?

Frail as we are, – we do know how to guide an infant into this world and how to care for it, love it, and bond with it...if we are just given the chance!
